Everything you need to prepare for your appointment, understand your care, and connect with our team—organized in one convenient place.

Please bring a photo ID, insurance card, current medication list, pharmacy information, and any relevant medical records or test results.
Please arrive 15 minutes before your scheduled appointment to complete check-in and any necessary forms.
If you have outside records, imaging reports, operative notes, or recent laboratory results, please send them before your appointment when possible.
Do not stop any prescribed medication unless your clinician specifically instructs you to do so. Bring an up-to-date medication and allergy list.
For routine office visits, usually not. If a procedure, sedation, or medication may affect driving, our team will give you specific transportation instructions in advance.
